Megan F. Butler, M.D.
Family Medicine – Algiers Clinic
Dr. Megan Butler earned her medical degree from LSU Health Sciences Center-School of Medicine. She completed residencies in Family Medicine in the East Jefferson General Hospital Family Medical Residency Program and the LSU Rural Family Medicine Residency program. Before joining Primary Care Plus, Dr. Butler was a staff physician at CareSouth Medical and Dental in Baton Rouge, Ochsner Health Center in Gretna, and LSU Health in Baton Rouge. She specializes in management of common chronic medical conditions such as Diabetes, Hypertension, Osteoarthritis and Substance Dependence.
Lacee Cavalier, FNP
Family Medicine – Houma Clinic
Lacee Cavalier earned a bachelor’s degree in nursing from Louisiana College in 2008. In 2016, she earned a master’s degree in nursing from Nicholls State University.
Lacee is a licensed family nurse practitioner and has over 14 years of experience in the medical field. Lacee previously worked for Preferred Health and Wellness, now Primary Care Plus – Houma.
MIGUEL A. CULASSO, M.D.
Internal Medicine – Slidell Clinic
Dr. Miguel Culasso earned a Doctor of Medicine in 1971 from the National University of Córdoba in Córdoba, Argentina. He completed an internship at the United States Public Health Service Hospital in New Orleans in 1974 and a residency in internal medicine at the Hospital Iturraspe in Santa Fe, Argentina, in 1975. Additionally, Dr. Culasso served as an emergency room physician at Charity Hospital in New Orleans, where he later completed his residency in emergency medicine in 1977.
Following completion of his residency, Dr. Culasso entered private practice. He has been practicing internal medicine in the St. Tammany parish area for more than 40 years.
Dr. Culasso is board-certified in emergency medicine.
DAPHNE J. DUREAU, FNP-BC
Family Medicine – Slidell Clinic
Daphne Dureau is a nurse practitioner who specializes in family medicine. She earned a Bachelor of Science in Nursing from Dallas Baptist University and a Master of Science in Nursing from Loyola University New Orleans.
Prior to joining Primary Care Plus, Daphne served as a family nurse practitioner at Family Heath Care & Wellness Center in Hammond, and from 2016 through 2019 she served in a number of roles at Benchmark Urgent & Family Care in Metairie, including research director and provider, manager of urgent care, assistant director of clinical trials and quality control coordinator. She also worked at Benchmark Research as the associate site director, nurse practitioner trial investigator and quality control coordinator.
Daphne is a board-certified family nurse practitioner. She’s a member of the Association of Clinical Research Professionals (ACRP) and a former president and current board member of the Southeast Louisiana ACRP chapter.
PHILLIP H. EHLERS, M.D.
Family Medicine – Baton Rouge – O’Neal Clinic
Dr. Phillip Ehlers earned his medical degree from St. George’s University School of Medicine in St. George’s, Grenada, and completed his medical education at the Louisiana State University Health Sciences Center Family Medicine Residency Program in Lake Charles, LA. Dr. Ehlers has been in practice since 2009 in Walker, LA; first with the North Oaks Health System and most recently with the Our Lady of the Lake Physician Group. His medical experience also includes practicing as an Urgent Care and Emergency Medicine Physician in the surrounding communities. Dr. Ehlers is a member of the American Academy of Family Physicians, as well as a Board Member of the Louisiana Academy of Family Physicians.

Stephanie Johnson, APRN, FNP-C earned an Associate of Science Nursing Degree from McNeese State University in Lake Charles, La. She continued her academic career, earning a Bachelor of Science Nursing Degree from Our Lady of the Lake College in Baton Rouge, La. furthering her education; she earned her Masters of Science in Nursing Degree specializing in family health nursing from Southern University A and M College in Baton Rouge, La.
Accomplished and eclectic nursing professional with 19 years of nursing experience. Well-known for keen diagnostic skills, excellent rapport, and top-notch patient advocacy. Stephanie has received her certification from the American Academy of Nurse Practitioners. Backed by ACLS and BLS Certifications.
RENE a. LOUAPRE, iii, m.d.

Dr. Rene Louapre earned a Doctor of Medicine in 1973 from Louisiana State University School of Medicine in New Orleans. He completed a residency in internal medicine at Charity Hospital in New Orleans in 1976.
Following completion of his residency, Dr. Louapre entered private practice. He has been practicing internal medicine in the New Orleans area for more than 40 years.
Dr. Louapre is board-certified in internal medicine.
JAIME MACALUSO, FNP-C
Family Medicine, Metairie Clinic
Jaime Macaluso earned an Associate Degree in Nursing from Charity School of Nursing in 2010. She continued her education at Louisiana State University Health Sciences Center, earning a Bachelor of Science in Nursing in 2012 and a Master of Science in Nursing in 2016. She had previously graduated in 2008 from the University of New Orleans with a Bachelor of Science in psychology.
Prior to joining Primary Care Plus, Jaime served as a family nurse practitioner, evaluating, diagnosing and treating acute and chronic illnesses in primary care and urgent care settings at East Jefferson General Hospital, JenCare Senior Medical Centers and The Urgent Care. She began her career in 2010 at East Jefferson General Hospital serving as a registered nurse in a medical-surgical unit.
Jaime is a member of the Sigma Theta Tau International Honor Society.
VAUGHN R. MEINERS, JR., M.D.
Family Medicine – Baton Rouge – Perkins Clinic
Dr. Vaughn Meiners completed medical school at Louisiana State University Health Sciences Center in Shreveport, and then continued his medical education at St. Vincent’s Family Medicine Residency in Jacksonville, FL. Before joining Primary Care Plus, Dr. Meiners spent 8 years in private practice as co-owner of Fountain Family Medicine in Jacksonville, FL, with his wife, Dr. R. Kim Meiners. Dr. Meiners is board certified in Family Medicine and has served as principal investigator on several research studies. He has a special interest in men’s health, diabetes management and cardiovascular risk management, including treatment of lipid disorders and hypertension.

Hector Montalvo, M.D.
Internal Medicine – Metairie Clinic
Dr. Hector Montalvo earned a Doctor of Medicine from San Carlos University School of Medicine in Guatemala in 1986. He participated in several clinical research projects at Tulane University School of Medicine, before completing post-graduate training at the University of Alabama at Birmingham.
Prior to joining Primary Care Plus, Dr. Montalvo served the Metairie community in private practice for 20 years, and he has been published five times.
Frank J. Monte, M.D.
Internal Medicine, Metairie Clinic
Dr. Frank J. Monte earned a Bachelor of Science in mathematics in 1973 and began pre-med courses at the University of New Orleans that same year. Dr. Monte completed studies at LSU School of Medicine and began an internal medicine residency at Charity Hospital in 1978. He earned board certification in 1981, following his residency, and accepted a faculty position that year as a clinical instructor in internal medicine at LSU School of Medicine, where students shadowed him in his office. Dr. Monte continued this role in a volunteer capacity until 2005.
Dr. Monte entered private practice after his residency and held staff appointments at Mercy Hospital, Memorial Medical Center, East Jefferson General Hospital and Doctors Hospital of Jefferson. In 1996, he was named a Fellow of the American College of Physicians in recognition of academic, educational, clinical and community service.
HANNAH PHAM, NP
Family Medicine – Napoleon Avenue Clinic
Hannah Pham earned a Bachelor of Science in Nursing in 2015 from Louisiana State University Health Sciences Center in New Orleans. Continuing her education there, she went on to earn a doctoral degree as a family nurse practitioner.
Hannah began her career as a registered nurse and has experience in ambulatory and transplant care. Prior to joining Primary Care Plus, she served as a nurse practitioner in outpatient settings with a focus on primary care and integrative medicine.
Hannah is board certified through the American Association of Nurse Practitioners. She values developing strong relationships with patients to provide the highest quality care.
DEANNA QUINN-STEVENSON, APRN, FNP-C
Family Medicine, Napoleon Avenue Clinic
Deanna Quinn-Stevenson began her career as a registered nurse in 1995. She earned her Master of Science in Nursing from LSU Health Sciences Center and has worked as a board-certified family nurse practitioner in the greater New Orleans area since 2005.
Deanna provides a range of diagnostic primary care services, including patient screenings, testing and exams. She also treats acute, episodic and chronic illnesses, and offers consultations about preventive health and treatment plans. Before joining Primary Care Plus, Deanna worked at local facilities, including University Medical Center – LCMC Health, West Jefferson Medical Center, CVS Minute Clinic and Correct Health, LLC.
BRAD J. RAMSEY, D.O.
Family Medicine – Baton Rouge – Perkins Clinic
Dr. Brad J. Ramsey earned his doctoral degree from Edward Via College of Osteopathic Medicine in Blacksburg, VA, and completed his internship and residency at North Mississippi Medical Center in Tupelo. He served as an attending physician at Wayne General Hospital, also in Mississippi, for just under two years before joining the Valley Emergency Physicians Fellowship in Walnut Creek, CA. In 2016, Dr. Ramsey returned to his home state of Louisiana. Prior to joining Primary Care Plus, he split his time between working as an urgent care physician at Lake After Hours in Baton Rouge and as an emergency physician at King’s Daughters Medical Center in Brookhaven, MS. Dr. Ramsey is a member of the American Academy of Family Physicians, the Mississippi Academy of Family Physicians, and the Mississippi State Medical Association. He holds certifications in advanced life support and critical care.
JERYL REISER-PARMENTER, D.O.
Family Medicine – New Orleans - North Broad Clinic and Slidell Clinic
Dr. Jeryl Reiser-Parmenter earned a Doctor of Osteopathy degree from Kirksville College of Osteopathic Medicine in 1993 and completed her family practice residency at Mesa General Hospital in 1998. She also holds a Bachelor of Science in Nursing from Northeast Missouri State University, which she earned in 1983.
Dr. Reiser-Parmenter has served as a physician for more than 20 years. Prior to joining Primary Care Plus, she served as medical director for PACE Greater New Orleans, responsible for oversight of hospitalizations and coordination of care. Dr. Reiser-Parmenter is board certified in family practice by the American Board of Osteopathic Medical Examiners.
Felicia Robertson, MSN, APRN, FNP-C
Family Medicine - Napoleon Clinic
Felicia Robertson earned a Bachelor’s of Science in Nursing degree from the University of Southern Mississippi in 2012 and received her Master’s of Science in Nursing degree from Southeastern Louisiana University in 2017. Felicia has been practicing as a registered nurse in the New Orleans metro area for 8 years and as a family nurse practitioner for the last three years. Felicia has worked in many fields over the years including urology, physical medicine, and geriatric care, but her passion has always been primary and preventative care. Her favorite part about being a nurse practitioner is the ability to take time to educate her patients on preventative health measures and making sure her patient’s healthcare priorities are heard and advocated for.

Completed undergraduate studies at the University of Central Arkansas in 1994. Worked in Geriatrics. Moved to New Orleans from Little Rock, Arkansas in 2002. Continued to work in Geriatrics at that time. Returned to school in 2005 and received a Master’s in Science in Healthcare Systems Management from Loyola University New Orleans in 2007. Continued to work in Geriatrics in a Management position, but decided working directly with patients was more satisfying and returned to school in 2010 and graduated with a Master’s of Science in Nursing with an Adult focus in 2012.
STEPHANIE L. SARRAT, M.D.

Dr. Stephanie Sarrat earned a Doctor of Medicine in 1997 from Louisiana State University School of Medicine in New Orleans and completed an internship and her residency in internal medicine, also at the university, in 2000.
Following completion of her residency, Dr. Sarrat entered private practice. She has been practicing internal medicine in the New Orleans area for nearly 20 years.
Dr. Sarrat is board-certified in internal medicine.
SARAH SCHWERTNER, M.D.
Family Medicine – Metairie Clinic
Dr. Sarah Schwertner earned Bachelor of Science degrees in both pre-med psychology and biological studies from the University of New Orleans before beginning LSU School of Medicine in 2008. She joined the Louisiana Army National Guard in 2010, where she performed health assessments for National Guard members. Dr. Schwertner completed her family medicine residency program at East Jefferson General Hospital in 2016.
Before joining Primary Care Plus, Dr. Schwertner provided urgent care, occupational medicine and family medicine in private practice across the New Orleans area. She previously held appointments at NOLADoc.com, East Jefferson Family Practice and Pelican State Outpatient Center.
Major Schwertner continues to serve in the National Guard as part of the U.S. Army Medical Command. She is board-certified and has been a member of the American Association of Family Physicians since 2013.
Ben Stanly, MSN, APRN, FNP-C
Family Medicine - Baton Rouge - Perkins Clinic
Benjamin Stanly is a Nurse Practitioner who collaborates closely with Dr. Vaughn Meiners, Jr. and Dr. Brad Ramsey. He received a Bachelor of Science in Nursing (BSN) degree from Northwestern State University of Louisiana. After earning his BSN, he worked as a registered nurse in critical care, progressive care, and urgent care settings. He later went on to complete his Masters of Science in Nursing (MSN) degree with a concentration in Family Nursing Practice at the University of Louisiana at Lafayette. Benjamin is Board Certified through the American Academy of Nurse Practitioners and licensed in the state of Louisiana. He is also a member of several professional and academic organizations, including the American Association of Nurse Practitioners (AANP), American Nurses Association (ANA), and Louisiana Association of Nurse Practitioners (LANP).
ANDREA L. STIRGUS, D.O.
Family Medicine - Westwego Clinic
Dr. Andrea L. Stirgus earned a Bachelor of Science degree in biology from Xavier University of Louisiana in 2009 before attending a pre-health program at Southern Illinois University. She graduated with a doctorate in osteopathic medicine from Ohio University Heritage College of Osteopathic Medicine in 2017. After completing her residency at Summa Health Family Medicine Center in Akron, Ohio, she received specialized training in geriatric medicine during her fellowship.
Dr. Stirgus began working with seniors in 2009 as a certified nursing assistant and worked in health care roles as often as her studies allowed before her residency. Before joining Primary Care Plus, she treated acute and chronic patient conditions in an underserved, urban population with a focus on older patients at the Summa Health Family Medicine Center.
Dr. Stirgus is board-certified in family medicine and board-eligible in geriatric medicine. She has been a member of the American Osteopathic Association since 2013, American Geriatric Society since 2015, and the Society for Post-Acute and Long-Term Care Medicine since 2020. During her time with Summa Health, she was awarded the Family Medicine Leadership Award and Geriatric Fellow of the Year award.
Thaddeus Temple, M.D.
Internal Medicine – New Orleans – North Broad Clinic
Dr. Temple earned a Bachelor of Arts in 1978 from Southern University at New Orleans and a doctor of medicine in 1983 from Louisiana State University Health Sciences Center New Orleans. In 1986, he completed his residency in internal medicine at Charity Hospital and at Veterans Affairs Hospital. Before joining Primary Care Plus, Temple served in the National Health Service Corps for four years, worked at the City of New Orleans – Saint Bernard Health Clinic for 13 years and practiced internal medicine in the Gentilly area for more than 30 years.
M. GEORGE THOMAS, M.D.
Cardiology – Slidell Clinic
Dr. George Thomas earned his Bachelor of Medicine-Bachelor of Surgery degree from St. John’s Medical College in Bangalore, India. After graduation he completed residencies both in India and at the New Rochelle Department of Medicine in New York. He completed his cardiology fellowship at Tulane University School of Medicine in 1983 and began teaching at the university.
Dr. Thomas joined the staff at Tulane University Medical Center in 1984. During that time, he also began working at Veterans Administration Medical Center and Charity Hospital of New Orleans.
In 1993, Dr. Thomas started Northlake Cardiology Associates, Inc. in Slidell. He spent nearly 28 years there and retained privileges at Northshore Regional Medical Center, Slidell Memorial Hospital, Lakeview Regional Medical Center and Crosby Memorial Hospital in Picayune, Miss. Dr. Thomas joined Primary Care Plus in 2021.
Dr. Thomas is board-certified in internal medicine and in cardiovascular disease with privileges at Slidell Memorial Hospital and Ochsner Medical Center – Northshore. He is a fellow in the American College of Cardiology, a member of the Musser-Burch Society and past president of the St. Tammany Parish Medical Society.
Candita Thompson, APRN, FNP-C
Family Medicine - North Broad Clinic
Candita R. Thompson is a family nurse practitioner who specializes in family medicine. She has a background in pediatrics, community health, nephrology and geriatrics. She completed her Bachelors of Science in Nursing at Grambling State University in Grambling, Louisiana and then earned her Masters of Science degree at Southern University A and M in Baton Rouge, LA. She maintains a national board certification with the American Credentialing Center (ANCC) as a family nurse practitioner. She has practiced nursing for a total of 26 years, 15 of those years in the nurse practitioner role. She has been employed with Primary Care Plus since December 2018.
Cynthia Tillman, APRN, FNP-C
Family Medicine - O'Neal Clinic
Cynthia Tillman is a nurse practitioner who specializes in family medicine. She has a background in a variety of areas, including geriatric care, retail health, primary care and emergency room trauma nursing. She earned a Bachelor of Science in Nursing from Louisiana State University Health Sciences Center New Orleans in 2001 and a Master of Science in Nursing in family practice from Loyola University New Orleans in 2011.
Cynthia has been a board-certified family nurse practitioner since 2011.
JOHN H. VOCKROTH III, M.D.
Internal Medicine – Algiers Clinic
Dr. John Vockroth earned a Bachelor of Science in biology from the University of Scranton and a Doctor of Medicine from the Medical College of Pennsylvania. He completed a residency in internal medicine at Madigan Army Medical Center in Fort Lewis, WA. Following completion of his residency, he served as staff internist with Ireland Army Community Hospital in Fort Knox, KY. Dr. Vockroth joined Primary Care Plus in 2013. Dr. Vockroth is board certified in internal medicine. He is a member of the American College of Physicians, the Louisiana State Medical Society and the Jefferson Parish Medical Society. Dr. Vockroth is a participating provider for Medicare, Peoples Health and most health insurance plans.
CRAIG WADE, M.D.
Internal Medicine – Houma Clinic
Dr. Craig Wade earned a bachelor’s degree in biology from The University of Southern Mississippi. In 2011, he earned a Doctor of Medicine degree from Ross University School of Medicine in the Commonwealth of Dominica.
Dr. Wade completed a residency in internal medicine at Leonard J. Chabert Medical Center in Houma. He then worked at Terrebonne General Medical Center, before opening Preferred Health and Wellness (now Primary Care Plus – Houma) in 2019.
Dr. Wade is board-certified in internal medicine. He is the winner of multiple Vitals Patients’ Choice Awards and Compassionate Doctor Awards.
CHEYENNE WELLS, NP
Family Medicine – Slidell Clinic
Cheyenne Wells earned her undergraduate nursing degree from Pearl River Community College in 2016. She then earned a Master of Science in Nursing from University of Mississippi Medical Center in 2020.
Cheyenne began her career as a registered nurse in Forrest General Hospital’s intensive care unit in Hattiesburg, Mississippi. Prior to joining Primary Care Plus, she served as a nurse practitioner at Solace Hospice in Poplarville, Mississippi.
Cheyenne is a board-certified family nurse practitioner and a member of the American Association of Nurse Practitioners. She develops strong relationships with her patients and their families to provide high-quality care.
